This form is used by our monitoring station to 
NOTIFY in the event of an alarm/fire signal, low battery, power outage, communication failure, etc.


Please complete the contact list in the 
exact order to be notified.

Three contacts are recommended, you MUST include yourself for your own account. 

**Passwords CANNOT contain any part of your account number and can only contain up to 12 characters and CANNOT include any symbols**


Each person on the contact list must have the correct account number and valid password for the account, access to the premise and have knowledge how to use the system in the event authorities need a keyholder to meet them at the premise.


*First 2 contacts will be set up automatically for email and text message notifications on all trouble signals. If you do NOT want text messages put NA in carrier box.

** TROUBLE SIGNALS ARE: LOW BATTERY, POWER FAILURES & COMMUNCATION FAILURES **

**We are only able to notify two of your contacts via email and/or text**

**We will need your Phone Carrier Name to set up Texting with our Monitoring Station**

(example AT&T/Verizon/T-Mobile)

Line of Security/Commerical Fire Waiver


Your monitored alarm system relies on your telephone line or internet connection to send the alarm signal to our central station. In the event that your phone line or internet connection is disabled your security system will not be able to communicate with our central station. If this occurs, no one will be notified of the alarm systems breach. Line security is an available option in the form of a cellular backup unit, which requires an additional installation cost and monthly fee. The customer is responsible to provide a reliable means of transmission of the alarm signal to our central station. All VOIP(Voice over IP) systems are not a reliable means of transmission.
